NYS requirements

New York State Requirements for students entering

     6 - 12 grade


All new students have 30 days from the first day of school to submit a physical exam from your doctor to the Health Office (a physical within the past 12 months is acceptable), OR you must call the Health Office with the scheduled appointment date.

Grades 7 and 10 are required to have a Physical Exam on record.
All Sport Physicals must be done by our School Physician

 RECORD  DTaP/DTP/Tdap: 4 to 5 DOSES (Diptheria, Tetanus, Pertussis Vaccine)
   IPV: 3 to 4 DOSES  (Polio vaccine)
   MMR: 2 DOSES  (Measles, Mumps, Rubella vaccine)

Tdap: 1 DOSE entering 6th -12th (Tetanus, Diptheria, Acellular Pertussisbooster)


MENINGOCOCAL: 1 DOSE entering 7th and booster entering 12th
( Meningococcal Vaccine)


Vision, Hearing Screenings will be done on 7th and 10th graders,
Scoliosis Screening will be done on all 6-9 graders

 DENTAL EXAM  Is requested for students entering, 7th and 10th grades

Other Forms you will need to submit


1) School provided Authorization for health information release form
2) School provided Health history form
3) School provided OTC Medication form
4) List of any medications the child is taking


* You must be compliant with NYS Immunizations Requirements before school entry.

Students who are not fully immunized must have with an appointment date scheduled for the

immunization or they will be excluded from school. Medical exemptions to immunizations must be

reissued yearly Serologic proof of measles, mumps, rubella, hepatitis B, varicella or polio antibodies is

acceptable proof of immunity. Diagnosis by a doctor that a child has had varicella is acceptable proof of
